How to Upload Images of your Medical Insurance Card to your SleepTest Patient Portal

While each operating system is different, we have some tips that may help! You may also reach out to your referring provider and request that they submit the images and the referral on your behalf. Note, Sleeptest.com cannot accept any images sent via email. Images must be uploaded to your SleepTest Patient Portal by you or your referring provider.

If on a Smartphone or tablet:

Open the camera on your phone
Take a picture of the front of your medical insurance card(s)
Take a picture of the back of the medical insurance card(s)
Close the camera app on your phone
Open your SleepTest Patient Portal
Click the "Click Here to Proceed" button
Follow the prompts to enter and/or confirm all necessary information
Where it asks, "Will you be using your medical insurance?" click the bubble for Yes
Tap or select the white box with the "+Click here to capture a picture of the front & back of card"
Select the images you took of the front and back of your medical insurance card(s)
If you have a secondary medical insurance provider, upload those images as well
Click the green Submit button

If on a computer or laptop:

Upload images of the front and back of your medical insurance card(s) to your desktop

Click the "Click Here to Proceed" button
Follow the prompts to enter and/or confirm all necessary information
Where it asks, "Will you be using your medical insurance?" click the bubble for Yes
Click the white box with the "+Click here to capture a picture of the front & back of card"
Select the images you took of the front and back of your medical insurance card(s)
If you have a secondary medical insurance provider, upload those images as well
Click the green Submit button
